This form is used for ordering of materials and for paying program fees.
Parishes/congregations that decide to sponsor the ministry complete
and submit this form.
All persons in leadership positions complete and
submit this form. This applies to
all members of parish/congregation steering committees (including leaders and
co-leaders), board of directors, regional advisory council, regional
coordinating groups, program committees, and other persons specified by the
board.
This form is completed and submitted for each program presentation several months prior to presentation.
This form is to be complete and submitted immediately following each
program
presentation.
This form is a sample for a program presentation.
This is an example of a Roster that is prepared and submitted
following each program presentation.
This checklist is useful in preparing and submitting the Roster.
This form is used for events where photographs, audio, video, and other "works" are being taken and recorded for future use by the ministry. Each individual / couple attending completes and signs a form. It releases and assigns the "work" to the ministry.
This form is used for copyright and trademark protection. Persons or organizations that create "works" (authoring, editing, photography, videos, etc.) for the ministry use this form. It releases and assigns the "work" to the ministry.
